A tremor is an involuntary, somewhat rhythmic muscle contraction and relaxation involving oscillations or twitching movements of one or more body parts. It is the most common of all involuntary movements and can affect the hands, arms, eyes, face, head, vocal folds, trunk, and legs. Most tremors occur in the hands. In some people, a tremor is a symptom of another neurological disorder.

Essential tremor (ET), also called benign tremor, familial tremor, and idiopathic tremor, is a medical condition characterized by involuntary rhythmic contractions and relaxations (oscillations or twitching movements) of certain muscle groups in one or more body parts of unknown cause. It is typically symmetrical, and affects the arms, hands, or fingers; but sometimes involves the head, vocal cords, or other body parts. Essential tremor is either an action (intention) tremor—it intensifies when one tries to use the affected muscles during voluntary movements such as eating and writing—or it is a postural tremor, which occurs when holding arms outstretched and against gravity. This means tha…

Clinical Reasoning: A 79-Year-Old Man With Subacute Onset Involuntary Facial Movements and Discoordination.. A 79-year-old man with metastatic Merkel cell carcinoma undergoing cancer-directed treatment was admitted to the hospital with 1 week of bilateral upper and lower extremity tremor and nearly continuous involuntary facial movements. These abnormal movements of his face, head, and limbs were exacerbated by purposeful movement. Examination revealed dysarthria with tremulous speech, intention tremors of the head and all extremities, distally diminished vibration sense with hyporeflexia, bilateral upper extremity dysdiadochokinesis, bilateral lower extremity dysmetria on heel-to-shin test…

Dyskinesia is a category of movement disorders that are characterized by involuntary muscle movements, including movements similar to tics or chorea and diminished voluntary movements. Dyskinesia can be anything from a slight tremor of the hands to an uncontrollable movement of the upper body or lower extremities. Discoordination can also occur internally, especially with the respiratory muscles, and it often goes unrecognized. Dyskinesia is a symptom of several medical disorders that are distinguished by their underlying causes.

Ocular tremor (ocular microtremor) is a constant, involuntary eye tremor of a low amplitude and high frequency. It is a type of fixational eye movement that occurs in all normal people, even when the eye appears still. The frequency of ocular microtremor has been found to range from 30 Hz to 103 Hz, and the amplitude is approximately four thousandths of a degree.

Definition and Classification of Involuntary Movements. Involuntary movements, also called hyperkinetic movement disorders , are defined as abnormal, unintended movements. They are defined as “abnormal” because not all unintended movements are pathological (reflexive or spontaneous movements also occur but are part of normal function; these include breathing, yawning, blinking, and the like). Involuntary movements can be classified into several categories according to their phenomenological characteristics and then according to the etiology and/or pathophysiology for each kind of involuntary movement. Involuntary movements include tremor, chorea, ballism, athetosis, dystonia, myoclonus, dys…

Hydromorphone-induced choreiform dyskinesia in a patient with homozygous sickle cell disease: a reversible adverse effect.. Background Sickle Cell Disease (SCD) is a type of chronic hemoglobinopathy with vaso-occlusive pain crises that requires aggressive pain management with opioids.Hydromorphone is commonly used in the management of pain in patients with SCD, particularly when vaso-occlusive crises require prolonged opioid therapy. While opioid-induced neurotoxicity is a well-recognized complication, usually presenting as delirium, agitation, or myoclonus, choreiform dyskinesias are rare and only infrequently reported. New-onset involuntary movements in patients with SCD may be concerning…

A Case of Painful Legs and Moving Toes Syndrome Mimicking Restless Legs Syndrome.. Painful legs and moving toes syndrome (PLMT) is a neurological disorder characterized by leg pain accompanied by involuntary toe movements. It may be misdiagnosed as restless legs syndrome (RLS) due to overlapping clinical features. We report the case of a 69-year-old woman with a seven-year history of pain involving all toes of both feet that improved with standing or walking and worsened when lying down. She had no sleep disturbance and was unaware of involuntary movements. A trial of dopamine agonist therapy was ineffective. Careful observation revealed involuntary toe movements, supporting a clinical diag…

[Acute Chorea].. Chorea is a hyperkinetic movement disorder characterized by involuntary, irregular, unpredictable, and purposeless movements that may involve the face, trunk, and limbs, with variable intensity and speed. In childhood, acute-onset chorea represents a neurological emergency, as it may reflect potentially treatable underlying conditions. From a pathophysiological perspective, chorea is associated with an imbalance between the direct and indirect pathways of the basal ganglia, particularly involving the striatum, resulting in excessive facilitation of motor activity. Pediatric chorea can be broadly classified into hereditary and acquired forms, with acquired causes being the m…
